Witnesses at the crash site reported utter carnage, with blood and pieces of people’s bodies strewn across the entire. Both aircraft plummeted into a San Diego neighborhood, killing all 135 onboard on Flight 182, seven people on the ground, and both the Cessna pilots. It is therefore unlikely to be fortuitous that miscommunication (i.e., communication errors) features prominently in many aviation accidents. The first two accidents involved breakdowns in verbal communication between flight crewmembers the third, between flight crewmembers and air traffic control (ATC). The crew tried to straighten out the navigational difficulties by switching back and forth from VOR/DME information to automatic direction finding (ADF). They were instructed to enter the south pattern, but the aircraft proceeded on a path to the west.Ĭonfusion arose in reading correct distances on their distance-measuring equipment (DME), as their instruments showed distances in kilometers. The flight initiated a go-around but did not follow the missed approach procedure or instructions from air traffic control. When informed that they had overshot the runway, the crew seemed unaware. The plane runs out of fuel and crashes shortly after this final communication to ATC. At this time, it became clear to tower controllers that the flight crew was disoriented. The YAK-42 did not proceed along the path required by the approach procedure and missed the localizer course. Radio communications are a critical link in the ATC system. The flight was cleared by the approach controller to execute an ILS approach to runway 16. Communications problems played a major role in reports of both airspace violations and near mid-air collisions (NMACs), which occurred most frequently in Class D airspace during early- to mid-afternoon (1201-1800 hours).
